I frequently create a supplemental medical record as a tool for trainings I conduct. Unfortunately, I have been creating them from the same 2 medical record forms for years now, so it’s time to mix it up a bit. I am in search of electronic blank medical record forms for the following areas: Emergency, Forensics, L&D, and any adult primary care focus. 

If you have ones you would be willing to share, it would be much appreciated. Please send to jenifer.markowitz@gmail.com.  Again, these would be used strictly for training. Having a variety, though will allow me to give folks an array of potential examples, which would be fantastic since we know medical record forms are definitely not one size fits all. BTW, they do not need to be from North American institutions; for some of my military clients they occasionally encounter medical records from civilian hospitals in Europe and Asia.
